5. Review the Rates Disclosure and click the Continue button. 6. Select an account. 7. (Optional) Click the “Add Account” link to add another account. 8. Click the Continue button when you are finished. 9. If necessary add any additional applicants. 10. Review the additional services and click the Continue button. 11. Answer the required questions and click the Next question button to move to the next question. 12. When you are finished, click the Submit questionnaire button.
