Getting Started Retrieve a Forgotten Username

If you happen to forget your username, you can easily retrieve it from the Bank Midwest Login screen—no need to call us!

2 1. Click the “Forgot Username or Password” link.

2. Select “Forgot Username” using the drop-down and click the Submit button. 3. Fill out the Customer Verification Form with the required information, and click the Submit button. 4. Your username will then appear. Click the Continue button.
